You can fill out the following forms and save them on your computer. Then print, sign and send them in. They are in MS Word format except where noted.

Original Retailer Application (Download all in a Zip file) - All of the forms that need to be submitted in order for a store owner to become a first-time Lottery retailer.

W-9 (PDF)
This is the IRS Tax Identification Number form. It is included in any retailer's original application packet, but is also listed separately here in case the form needs to be re-filed.
EFT (Electronic Funds Transfer) Application with instructions
This is the form needed to give the Lottery permission to debit/credit entries to a retailer's designated bank account.
